What does chronic disease management entail?
We have an integrated care approach to chronic disease management. With patients, we schedule screenings and checkups regularly. We not only monitor and coordinate treatment with other doctors and specialists, but we also educate a patient on their chronic disease. Our goal is to reduce your health care costs and improve your quality of life by preventing or minimizing the effects of your illness.
Since there is currently no cure for asthma, we want to help prevent chronic shortness of breath and chronic coughing. While quick-relief asthma medicines are vital for the treatment of it, we work to reduce the need for them. Our doctors will work to help maintain good lung function so you can keep your normal, everyday activities and get a good night sleep. We want to prevent attacks that are so severe that it results in visits to the emergency room or even hospital stays.
We will develop an action plan with you to ensure you are taking your medication correctly and how to avoid what triggers your asthma. Also included will be regular education on recognizing symptoms and signs of worsening asthma and when to act promptly to seek medical attention. Finally, we’ll establish a schedule of regular Asthma Checkups depending on how well it is being controlled.
COPD (short for chronic obstructive pulmonary disease) management involves the following
- Disease monitoring
- Risk factor reduction
- Managing a stable COPD
- Managing exacerbations
The goal of our COPD management program is to prevent the disease from progressing further while relieving you of the symptoms. This involves improving your overall health status and exercise tolerance. Our doctors will help prevent and treat any complications or exacerbations. Overall, we want to reduce your mortality chances from this disease and also minimize the side effects of treatment. Every patient is different so the right treatment combinations will be individualized to that patient.
Currently, there is no cure for diabetes; however, decades of research has developed very successful ways to treat and control it. Overall, a diabetes management program is designed to keep your blood glucose levels as close to normal as possible. Ideally, this is done by balancing your food intake with medication and activity. We will also monitor your cholesterol and triglycerides (lipid) levels and try to keep them as near to normal ranges as possible. Your blood pressure will be monitored and managed to make sure it does not go over 140/90. We want to avoid or decrease the development of any diabetes-related complications.
To successfully manage diabetes we will plan a balanced meal plan and ensure you exercise regularly. If medication is needed, guidelines will be strictly followed. Together, we will monitor your blood glucose levels and blood pressure levels when you visit our clinic and also at home. Routine lab tests will be scheduled to ensure all of your levels are in line with your management plan.
